The first thing that you want to avoid is using expired baking powder and baking soda. Just like anything else, when you are cooking with either of the two products you will need to check the expiration date. If you do use expired baking soda and baking powder, the cookies will not rise as much as they should.

Not sticking to the recipe is another thing that you should avoid. When you are cooking based on a recipe, everything that is listed is important for the final product. If you leave out or switch certain ingredients, the cookies may not come together as they should.

Lastly, if you are cooking multiple trays of cookies, you need to make sure that they all cook evenly. The tray that is higher in the oven will cook a little bit faster than the one that is under it. With this in mind you will have to switch the trays midway through baking.
